Xarelto Linked to Blood Clots: ISMP
Xarelto is prescribed to many patients as a prophylaxis for the prevention of blood clots. Unfortunately, new research has identified a link between the drug and an increased risk of blood clots – particularly in younger individuals. According to an October 2012 report published in the Institute of Safe Medicine Practices (ISMP), Xarelto was associated with at least 356 “serious, disabling, or fatal injuries” during the first quarter of that year alone. The study’s authors specifically warned about the risk of Xarelto blood clots:
“The largest identifiable category was serious blood-clot-related injury — most frequently pulmonary embolism — the very events rivaroxaban is intended to prevent. … These thromboembolic events with rivaroxaban occurred in younger patients (median age 66 years).”
Blood Clot Symptoms
Hip or knee replacement patients usually have limited mobility for a period of time after surgery. The longer they remain immobile, the greater their risk of developing a deep vein thrombosis (DVT), or blood clot in the deep veins of the lower legs. DVT symptoms may include:
- Swelling, typically in 1 leg
- Leg pain, tenderness or cramping
- Reddish or bluish skin discoloration
- Leg warm to touch
Deep vein thrombosis is a life-threatening condition because these clots can break away from the implant site and become trapped in vital organs. If a DVT becomes trapped in the lungs, it is referred to as a pulmonary embolism (PE). Signs of this condition include:
- Sudden shortness of breath
- Sharp, stabbing chest pain that gets worse with deep breath
- Rapid heart rate
- Unexplained cough, sometimes with bloody mucus

JAMA Study Investigates Blood Clot Treatment Strategies
A September 2014 study published in the Journal of the American medical Association (JAMA) found that most treatment options for blood clots in veins (venous thromboembolism) were equally safe and effective, yet major bleeding events occurred more often in patients treated with anticoagulants like Xarelto. Researchers conducted 45 randomized trials on 44,989 test subjects, and found that 0.49% of Xarelto users suffered a major bleeding event during the first three months of treatment, compared to just 0.29% with Eliquis. The study’s authors concluded that, except for a UFH-vitamin K antagonist combination, all other treatment strategies had similar bleeding risks.
