What’s the Problem?
For Risperdal users with suspected neuroleptic malignant syndrome, treatment typically begins with supportive therapy, hydration and metabolite stabilization with cooling blankets to avoid hyperthermia. Ventilator assistance and dialysis may occasionally be used to treat renal dysfunction.
Drug therapy starts with the patient quitting Risperdal in favor of another antipsychotic with fewer potential side effects. There are several other drugs available, but the two most commonly used are dantrolene and bromocriptine.
Once symptoms subside, the patient should be taken off dantrolene but remain on bromocriptine. Treatment should last 10 days or more for Risperdal users. Other courses of treatment may include levodopa, pergolide, benzodiazepines and electroconvulsive therapy.
If neuroleptic malignant syndrome develops with Parkinson’s disease, treatment should be the same except that medications used to treat the Parkinson’s should be re-instituted right away. Due to the potential for NMS, drug holidays are not normally recommended for people with Parkinson’s disease.
If the patient is to resume treatment with an antipsychotic, a break of several weeks or more should be taken first. Risperdal should be avoided in favor of a drug with fewer potential side effects.
