What are the Symptoms of Neuroleptic Malignant Syndrome (NMS)?
- Mental status change – The principal symptom of NMS, which typically manifests as agitated delirium with confusion. Signs of catatonia and mutism may also be present. Progression to full-blown encephalopathy with stupor and coma is commonly reported.
- Muscular rigidity – Increased muscle tone that can be seen in the movement of the arms and legs. This symptom is often called “lead pipe rigidity,” or stable resistance throughout all ranges of movement. Accompanying tremors may cause a ratcheting quality or “cogwheel phenomenon.” Other motor abnormalities have been reported to include tremors, dystonia, opisthotonus, trismus, chorea, and dyskinesia. Patients may also experience sialorrhea, dysarthria, and dysphagia.
- Hyperthermia – According to many diagnostic criteria, hyperthermia is the defining characteristic of neuroleptic malignant syndrome. Temperatures of more than 100ºF are commonly reported, but even higher temperatures of over 104ºF have been seen in patients with NMS.
- Autonomic instability – Symptom that typically manifests as tachycardia, labile, elevated blood pressure and/or tachypnea. Dysrhythmia may also occur. In most cases, diaphoresis is widespread.
Most patients with Risperdal-induced neuroleptic malignant syndrome follow a course of mental changes appearing 1st, followed by rigidity, hyperthermia, and autonomic dysfunction. Some reports note a delay in the appearance of fever for a day or more, which leads to difficulty in diagnosing the condition. There is considerable variability in the presentation of NMS, and other cases do not necessarily indicate a typical course for the syndrome.
