What is Rexulti?
Rexulti (generic: brexpiprazole) is an oral atypical antipsychotic drug used to treat schizophrenia, and as an add-on treatment for adult patients with major depressive disorder (MDD). The medication works by rebalancing dopamine and serotonin to improve thinking, mood, and behavior. Other atypical antipsychotics in the same class as Rexulti include Abilify (aripiprazole), Zyprexa (olanzapine), Seroquel (quetiapine) and Geodon (ziprasidone).
Rexulti vs Abilify: How Different Are They?
Both Rexulti and Abilify have very similar pharmacological profiles. Both are partial agonists at dopamine D2 receptors, the mechanism of action responsible for the drugs’ antipsychotic effects. Both also activate serotonin 5HT1A receptors and block 5HT2A receptors.
So what’s the difference between Rexulti vs Abilify? The main difference, at least according to critics, is that Abilify is off-patent (generic) and is therefore far less profitable than a new medication that has patent protection. Otsuka America Pharmaceutical has been accused of evergreening Abilify and Rexulti, a marketing strategy pharmaceutical companies sometimes use to continue generating massive profits once a drug has gone off-patent.
FDA Study on Rexulti and Compulsive Behaviors
Compulsions are a rare but serious side effect of Rexulti, according to a recent study by the FDA. Among 2,627 participants who reported side effects from Rexulti, at least 3 suffered from compulsive behavior. All 3 patients had been taking the medication for 6 months or less at the time symptoms first appeared, according to the researchers.

Serious side effects of Rexulti may include:
- Uncontrollable Compulsive Behaviors – Gambling, sex, shopping, eating.
- Extrapyramidal Effects – Acute dyskinesias and dystonic reactions, tardive dyskinesia, Parkinsonism, akinesia, akathisia, neuroleptic malignant syndrome.
- Suicidal Thoughts – Particularly in children, teenagers and young adults.

- Weight gain
- Runny / stuffy nose
- Increased appetite

Not approved for dementia-related psychosis; incr. mortality risk in elderly dementia pts on conventional or atypical antipsychotics; most deaths due to cardiovascular or infectious events; extent to which incr. mortality attributed to antipsychotic vs. some pt characteristic(s) not clear.
Incr. suicidality risk in children, adolescents, and young adults w/ major depressive or other psychiatric disorders; weigh risk vs. benefit; in short-term studies of antidepressants vs. placebo, suicidality risk not incr. in pts >24 yo, and risk decr. in pts 65 yo and older; depression and certain other psychiatric disorders themselves assoc. w/ incr. suicide risk; observe all pts for clinical worsening, suicidality, or unusual behavior changes; advise families and caregivers of need for close observation and communication w/ prescriber; not approved for depression in peds pts.
Should I Stop Taking Rexulti?
