What is Proquin XR?
Proquin XR (generic: ciprofloxacin) is an antibiotic that belongs to a group of medications called fluoroquinolones. The drug is used to treat uncomplicated urinary tract infections (acute cystitis), and works by fighting bacteria in the body. Proquin XR is made by Esprit Pharma, and was approved by the U.S. Food & drug Administration (FDA) in May 2005.
Tendonitis, Tendon Rupture ‘Black Box’ Warning
In 2008, FDA required the makers of fluoroquinolone antibiotics to include a ‘black box’ warning regarding tendon ruptures to the medications’ labeling. Ruptures and tears associated with fluoroquinolone use were reported in the Achilles tendon (most prevalent), rotator cuff, hand, bicep and thumb. FDA’s Adverse Events Reporting System (FAERS) confirmed that tendon ruptures in fluoroquinolone users have continued to increase since the warning was issued.
Studies Link Fluoroquinolone Antibiotics to Aortic Damage
Recent studies have found that fluoroquinolone antibiotics may damage certain types of collagen. This may result not only in tendon rupture, but also aortic dissection and/or aortic aneurysm.
In October 2015, a study published in JAMA Internal Medicine found that current use of a fluoroquinolone antibiotic was linked to a 2.4-fold increased risk of aortic aneurysm, and past use was linked to a 50% increased risk.
Another study published in BMJ Open in November 2015 found that type I and type III collagen make up most of the collagen in the Achilles tendon, as well as most (80-90%) of the collagen in the aorta. The study’s findings suggested that “Diseases of the aorta, including aortic aneurysms and dissections, are associated with alterations in collagen content, concentrations, and structure.”
Because these collagen degradations are the likely cause of tendon injuries linked to fluoroquinolones, the study’s authors conjecture that such changes may lead to aortic aneurysm and aortic dissection as well.
What is an Aortic Aneurysm?
An aortic aneurysm is an enlargement or bulging of the aorta, the largest blood vessel in the body. Aneurysms occurs when a segment of the vessel becomes weakened and expands. The pressure of the blood flowing through the aorta creates a bulge at the weak spot, starting small and growing as the pressure continues. Aortic aneurysms are dangerous because they can rupture, causing fatal internal bleeding events.
Signs and symptoms of an aortic aneurysm may include:
- Abdominal or back pain
- Pain spreading from the abdomen or back to the pelvis, legs or buttocks
- Clammy or sweaty skin
- Increased heart rate
- Shock or loss of consciousness
Proquin and Nerve Damage (Peripheral Neuropathy)
In August 2013, FDA issued a Drug Safety Communication stating that fluoroquinolone antibiotics had been linked to a severe form of nerve damage called peripheral neuropathy. The agency issued the communication because earlier warnings did not adequately describe the potential rapid onset of the disorder.
“In some patients the symptoms had been ongoing for more than a year despite discontinuation of the fluoroquinolone,” FDA said. “Several patients were continued on the fluoroquinolone drug despite the occurrence of neuropathic symptoms.”
Peripheral Neuropathy Symptoms
Peripheral neuropathy occurs when nerves between the brain and body become damaged and are unable to effectively communicate with each other. There are more than 100 forms of the condition known, depending on which nerves have been damaged. Symptoms may include:
- Change in sensation to light touch, pain, or temperature
- Change in sense of body position
- Loss of reflexes
- Muscle wasting

- Muscle weakness (myasthenia gravis)

- Central nervous system disorders (tremors, restlessness, lightheadedness and confusion)
- Hypersensitivity reactions (loss of consciousness, cardiovascular collapse)
- Cardiovascular collapse
- Life-threatening skin reactions
- Toxic epidermal necrolysis (TEN)
- Stevens-Johnson syndrome (SJS)
- Clostridium difficile associated diarrhea (diarrhea caused by overgrowth of the bacterium C. difficile)
