PPI Heartburn Medicines Linked to Risk for Heart Failure, Death

Proton pump inhibitors have been linked to an increased risk for heart failure and death in patients with coronary artery disease, according to a new study.

What’s the Problem?

March 23, 2017 – The study, which was published Jan. 19 in the journal PLOS ONE, found that chronic antiplatelet therapy (aspirin, Plavix, Effient) in patients with coronary artery disease carries the risk for gastrointestinal complications including ulcers, hemorrhage and gastric erosions.

Proton pump inhibitors are commonly used to prevent or treat these complications; however, use of the drugs may cause pneumonia, micronutrient deficiencies, and osteoporosis-related fractures, according to the study.

PPIs include:

Limited data suggests that use of these medications could inhibit the antiplatelet effect of aspirin and clopidogrel and lead to more cardiovascular events in CAD patients.

For the study, researchers looked at a cohort of 706 patients, 431 of whom took proton pump inhibitors (most commonly omeprazole). Approximately two-thirds of the patients were taking clopidogrel, and over 90% were taking aspirin.

The study found that PPI use increased the risk for all-cause heart failure or death (HR, 5.713; P =.007), but not for acute ischemic events.

“In conclusion, our results suggest an association between use of PPIs and the incidence of death or [heart failure] but do not point to an association with incidence of acute ischemic events,” the researchers concluded. “More studies are needed to confirm these data.”
